I acknowledge and declare that I have read this express consent text and the Disclosure Text provided to me separately, and I hereby accept, declare, and commit that I pre-approve the collection, recording, processing, and storage of my health data by ALTIN GÖK TURİSTİK TESİSLERİ TİCARET VE SANAYİ A.Ş. in accordance with the principles set forth in Law No. 6698 on the Protection of Personal Data.
